But behind that simple appearance is a carefully engineered system designed to move teeth in precise, predictable directions. The Building Blocks: Brackets, Bands, and Wires


Every set of braces is built from a few core components. Brackets are the small pieces bonded directly to the surface of each tooth, acting as anchors that hold everything else in place. Today's brackets are typically made from stainless steel or a tooth-colored ceramic material, depending on a patient's preference and treatment plan.

Running through the brackets is the archwire, a thin metal wire that connects the brackets to one another. This wire is the true workhorse of the system. Archwires are shaped and sized specifically for each stage of treatment, and many are made from materials with shape memory, meaning they naturally try to return to their original form. When bent to follow the current position of misaligned teeth, the wire continuously exerts gentle pressure as it attempts to spring back into its intended shape, pulling the teeth along with it.

Bands, which wrap around certain back teeth like molars, provide extra anchorage for more complex movements. Small elastic or metal ties, called ligatures, hold the archwire securely against each bracket, and colorful elastic ties are often what patients notice first since they can be swapped out at each visit.

The Role of Elastics and Additional Appliances


Beyond the basic bracket-and-wire system, many patients also wear rubber bands, often called elastics, that stretch between the upper and lower braces. These elastics apply targeted pressure to correct bite issues such as overbites or underbites, working the jaw into better alignment rather than just the teeth themselves. Some treatment plans call for additional appliances, such as expanders that widen a narrow palate or spacers that create room between crowded teeth before brackets are placed.

Each of these tools serves a specific mechanical purpose, and your orthodontist selects them based on exactly what a patient's teeth and jaw need to achieve proper alignment.

How Pressure Translates Into Movement


The mechanics of tooth movement come down to sustained, gentle force. Each tooth sits in a socket in the jawbone, cushioned by a thin layer of tissue called the periodontal ligament. When braces apply steady pressure in a particular direction, that ligament compresses on one side of the tooth and stretches on the other. The body interprets this as a signal to remodel the surrounding bone, breaking it down where it's compressed and rebuilding it where it's stretched, allowing the tooth to gradually shift into a new position.

This process is slow by design. Moving teeth too quickly can damage the root or surrounding bone, so orthodontists calibrate the amount of pressure carefully, using small, incremental adjustments rather than dramatic force. It's this deliberate pacing that allows braces to reposition teeth safely over the course of months or years.

What Happens at Adjustment Appointments?


Adjustment visits, which typically happen every four to eight weeks, are where the orthodontist fine-tunes the treatment. During these appointments, the orthodontist may tighten or replace the archwire, swap out elastic ties, or add new components as treatment progresses. Each adjustment nudges the teeth a little further toward their final position, building on the progress made since the last visit.

Patients often notice mild soreness for a day or two after an adjustment, which is simply a sign that the teeth are responding to renewed pressure. This discomfort is typically much milder than what many people expect and tends to fade quickly as the mouth adjusts to the new pressure. Sticking to soft foods and using a warm saltwater rinse in the days following an adjustment can help ease any temporary tenderness.

Between visits, the orthodontist tracks how each tooth is responding, comparing progress against the overall treatment plan mapped out at the start. This ongoing monitoring allows small corrections to be made along the way, rather than waiting until the end of treatment to notice that something isn't tracking as expected.

Why Treatment Happens in Stages


Orthodontic treatment isn't a single continuous push in one direction. It typically unfolds in stages, each with its own objective. Early stages often focus on resolving crowding and creating space, using progressively larger and stiffer archwires as treatment advances. Middle stages may address bite correction using elastics or other appliances. Final stages focus on fine-tuning tooth position and settling the bite before braces are removed.

This staged approach allows the orthodontist to address the most pressing issues first, then refine the results as treatment continues. Each stage builds directly on the work of the one before it, which is part of why consistent attendance at adjustment appointments matters so much for staying on track.

Clear Aligners as an Alternative System


Clear aligners rely on the same biological principle, gentle sustained pressure prompting bone remodeling, but apply it differently. Instead of brackets and wires, a series of custom-fitted plastic trays gradually shift teeth, with each new tray in the series applying slightly more pressure toward the next incremental position. Patients switch to a new tray roughly every one to two weeks, with each tray representing another small step in the overall treatment plan.

Because aligners are removable, they place more responsibility on the patient to wear them consistently, typically 20-22 hours a day, in order to keep treatment progressing on schedule.

Putting It All Together
